Please utilize the below checklist as a step-by-step requirements guide to get the most competitive proposal through the Alliance National Network Health Pan.

1. Census Template 

  • Please download and use the templated provided here. If you are wish to use an internal report please be sure to include all data points listed in the template.

2. Current/Renewal Plan Information 

  • Plan design (benefit summaries if possible) and rates for each plan offered.

3. Renewal Plan Options

  • Plan design(s) requested that are different than your current offering.

4. Organization Contributions 

  • ($ or %) for employee and dependents.

5. Objectives

  • What are your goals, preferences and risk tolerance? We use this to design a proposal.

6. Experience Reports

  • If 100+ employees enrolled please provide:
    • 2 years of carrier reporting that includes:
      • Active members enrolled by month
      • Medical claims by month
      • Rx claims by month
      • Large claimant information (list claimants with $25,000 or more in total paid claims)
        • Please include total paid claims, diagnosis, prognosis and expected ongoing cost.
  • If under 100 employees enrolled please provide:
    • Medical questionnaires, if no carrier reporting is available
    • If carrier reporting is available, 2 years (current and prior year)
